When you finally move into the area where the bomb is and secure it, have the person holding the Defuser activate it. If that player goes down someone else will need to pick it up. Since many players might be picked off when going for the diffuser, you can also eliminate the enemy team to win the round as well. So, even if you can’t get to the diffuser or the bomb site, you can still win by simply killing all five members of the opposing team. If you are able to activate the diffuser, this will kick off a 45 second timer in which your team must defend it, or kill all of the Attackers that are trying to stop you.
